I personally have never had any alterations done, but I have heard that there are several good places around town to get them. I've heard that both Hyde Park Tailors at Hyde Park Square and Mike Trotta Tailors in Downtown Cincinnati are both very good, if somewhat on the pricey side. A good, but more reasonably priced place I've heard of for alterations is Kim Dao's Alterations on Court Street in Downtown Cincinnati. Some local dry cleaners also offer alteration services, but I'm not sure how much I would trust them.
